Deductibles and copays apply and are part of our contracts with your insurance plan. Please see our billing policy for more information.
We are working to add more insurance plans. At this time there are a few that are not accepting new providers. We will continue to monitor for availability as we are striving to be available with all plans our patients need. You may also call your insurance company and request they add us to their network!

SELF-PAY PATIENTS
We understand that every patient is unique, including their financial circumstances. We offer a reasonable self-pay fee schedule for those who choose to pay up front for their medical visit and service.

LAB INFORMATION
Lab Contact Information
We utilize the lab facilities we believe to provide the best diagnostic tools for our patients. Please reach out to the lab directly with any billing concerns you receive from them. Please also make sure your correspondence is not simply an "Explanation of Benefits" or "EOB" from your insurance company rather than a billing statement from the lab directly. These "EOB's" are often not an accurate representation of what you will be financially responsible for. See list below for contact information for labs we utilize.
